Each reported incident is duly evaluated by the CARS staff and a patient safety work product (PSWP) is created. The PSWP can vary with the complexity of the reported incident. It can be simply be a memo to the reporting facility/provider acknowledging the receipt of the incident and recording of the incident in the relational database. Some incidents that meet the threshold for rigorous evaluation go through more comprehensive analyses (such as root cause analyses). In such cases, a written report is generated and communicated to the reporting facility/provider with recommendations for patient safety, health care quality, or health care outcome improvement.
